Statement of Information - D81 form
Nearlythere - 03 August, 2017 06:33PM
With regards to I confirm that I have read a fully completed statement for a consent order on the D81 form. I have received a fully completed Statement of information however it was not signed by the other party on the applicant's statement of truth page. Should I confirm that I have read a fully completed statement of information and sign this page or only sign this page once I have received a signed copy of the statement of truth page on his form.

Thank you
